Premature ejaculation (PE) means ejaculating sooner than you would like, typically within one to two minutes of penetration, before you feel ready and before you can control the timing. PE can be lifelong (always been that way) or acquired (something that has changed over time). It can happen in some situations and not others, or consistently every time. All of these patterns are recognised and all are treatable, so the picture you describe in the consultation helps the prescriber choose the right approach for you.
The exact causes are not fully understood, but PE most likely involves a combination of psychological and physical factors. Psychological contributors include performance anxiety, stress, relationship difficulties or simply a conditioned response built up over years. Physical factors include heightened sensitivity, neurological differences in ejaculatory reflex thresholds, and hormonal variation. Some medications can also affect ejaculatory timing.
For many men, anxiety about PE creates a self-reinforcing cycle — worrying about it makes it more likely, which increases the worry. Breaking that cycle, whether through behavioural techniques, medication, or both, is what treatment is designed to do.
Depending on what the prescriber finds, treatment might be a short-acting tablet taken a few hours before sex, a numbing cream applied beforehand to reduce sensitivity, or a combination of the two. Behavioural approaches — the stop-start technique, the squeeze technique, mindfulness — are effective for many men and work particularly well alongside medication.
